AT&T PREPAID PIN/transfer PIN (if the new carrier requires a PIN)
ZIP + name on file (match exactly)

Confirm the line is active
Do not cancel service before porting.
Get the account number + PIN
Have the customer log into AT&T PREPAID My Account to retrieve their account details: https://www.att.com/prepaid/
Submit the port
Enter: phone number, account number, PIN (if required), ZIP, name.
Common rejections (quick fixes)
PIN rejected: confirm the correct prepaid PIN and resubmit
Account # rejected: re-check the account number in Prepaid My Account
